Telehealth Consent

This Telehealth Consent (“Consent”) governs the telehealth services provided through the Everhuman platform, operated by Syndicate AI LLC d/b/a Everhuman, a Florida limited liability company. Our clinical providers are Arora Health & Aesthetics, LLC, located at 300 Lenora Street, Seattle, WA 98121. They can be contacted at medicalcompliance@arorahealthgroup.com. By using Everhuman’s services, you agree to the terms of this Consent.

1. What Is Telehealth?

Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a licensed healthcare provider and a patient who are not in the same physical location. Telehealth may be used for diagnosis, treatment, follow-up care, and patient education.

2. Clinical Provider

All clinical services, including provider consultations, medical evaluations, and prescription issuance, are performed by licensed providers affiliated with Arora Health & Aesthetics:

Everhuman provides an asynchronous (and synchronous when needed, for non-controlled substances) telemedicine platform that delivers your online visit information to our clinical providers and coordinates prescription fulfillment through our partner pharmacy.

Everhuman is a technology platform only and does not practice medicine, employ physicians, or control clinical decision-making.

3. Expected Benefits of Telehealth

Telehealth services may provide the following benefits:

Improved access to healthcare services without requiring in-person visits

Efficient evaluation and, where clinically appropriate, prescription of treatment

Ongoing communication with your licensed provider through secure digital channels

4. Potential Risks of Telehealth

As with any healthcare service, there are potential risks associated with telehealth, including:

Information transmitted may not be sufficient for a complete medical evaluation

Delays in evaluation or treatment due to technical failures

Security protocols may fail, resulting in a breach of privacy of personal health information

Lack of access to complete medical records may result in adverse drug interactions or allergic reactions

5. Your Rights

By consenting to telehealth services, you acknowledge and agree that:

You have the right to withhold or withdraw consent to telehealth services at any time without affecting your right to future care

All applicable laws protecting the confidentiality of your medical information apply to telehealth services

You have the right to inspect all information obtained and recorded during a telehealth interaction

Telehealth services are not a substitute for emergency medical care; if you are experiencing a medical emergency, call 911 immediately

6. Privacy and Security

We implement industry-standard security measures to protect your health information during telehealth interactions, including SSL/TLS encryption for all data transmissions. Your PHI is handled in accordance with HIPAA and our Privacy Policy.

7. Prescription and Pharmacy

Where clinically appropriate, your licensed provider may issue a prescription. Prescriptions are fulfilled by our licensed pharmacy partner:

Medications are compounded specifically for each patient pursuant to a valid patient-specific prescription. Compounded medications are not FDA-approved.

8. State Availability

Everhuman provides telehealth services to eligible adults in all 50 U.S. states. Care is provided by affiliated licensed providers, and prescriptions are fulfilled only through licensed pharmacy partners authorized to dispense in the patient’s state. Medication availability may vary by state based on provider licensure, pharmacy licensing, and applicable state telehealth requirements.

9. Emergency Situations

Telehealth services are not appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Do not use the Everhuman platform for emergency medical situations.
